Just a reminder that if you cannot attend a flu vaccination clinic, you can get your Flu Vaccine from a participating Pharmacy or from your GP.
All you need to do is submit your completed consent form (see attached) along with the vaccination record received from the GP/Pharmacy, to be included in the incentivised draw.
The process is outlined below.
